With the flight attendant's situation i was confused too. I ended up just thinking she used the chick to simply visualize which clothes to wear by using her, and it would explain why she cared about her weight too (in a way she can feel powerful and superior, in opposition of how she feels with her "friends"). Idk I might be wrong tho

To answer your questions... The old man was just a patient of the doctor's (the one who would go around asking people for a sip of their coffee). I think they were kinda close cuz the doctor would walk with him sometimes. Seems like he was just upset because he died and he wasn't there (maybe reminded him of his father's death)... For the flight attendant, I think that girl was her personal shopper and it's easier for her to buy things if they have the same body weight and type. The flight attendant wanted her to return the clothes back to the store since they wouldn't fit her. When she was talking about "performance review" she just meant she would give the girl a bad review for being a personal shopper, which would impact her getting another job after. I think it was also to show how self-conscious the flight attendant is about her appearance...
