One thing I'd say in response to the idea that Kirito hasn't really ever experienced failure is that failure for Kirito doesn't really mean him losing a fight or dying. He has constantly showed that he values other peoples lives over his own so in his mind, failure is failing to save the people who he cares about, or being forced to kill in order to do so which is something that has happened countless times (mostly in Aincrad). If you look at it that way he fails quite a lot, and through all of this he constantly blames himself for those failures. The failure to save Sachi and the moonlit black cats, the failure he felt when he was forced to kill several people in Aincrad to survive, Kirito's character is really defined by how he blames himself for those failures and his struggle to find forgiveness from those he failed to save.

You get it. That's what drives him to such extremes, why he finds the power to keep going and fighting, why he wants to create a VR world too, so that he can stop those things from happening. Also the possible reality addiction that he has, but it's all so deep in his psyche that he's constantly failing and trying to not do it again
