For those who are in interested reading along with the original Heavenly Delusion manga, this episode adapted the remaining 4 pages from chapter 6 (as the after-credits scene of this episode), all of chapters 8 and 9, and the first 14 pages of chapter 10 (14/32). This is equivalent to about ~2.5 chapters worth of manga adapted for episode 3, which marks a slowing in the pacing compared to the first two episodes (which averaged about ~3.5 chapters of manga per episode). Also if it wasn’t clear from this episode, we are indeed supposed to get the implication that Haruki had an incestuous crush on his older (biological as far as we know) sister Kiriko. Also the reason why Kiruko in the present goes by the name “Kiruko”, is because it is formed from a combination of the Japanese characters from Haruki and Kiriko’s names. Also if one keeps an eye out for details, they may have noticed during the scene where Kiruko is looking at themselves in the mirror (and we can see the surgery scars encircling their skull) one may notice a separate smaller surgery scar on the upper back part of Kiruko's skull, could that possibly be related to the gunshot-like sound we heard earlier when Haruki lost consciousness? Also if anyone needs a refresher on the characters ages, in 2034 Haruki was 13 years old and Kiriko was 15 years old, hence why Kiruko has described themselves as both 18 and 20 in 2039 (as both are sorta true). Also this episode not only gives further context to the mirror scene from episode 1, but also how Kiruko stated they had personal past experience of having almost been eaten by a Man-Eater/Hiruko before in episode 2. For the school kids Tokio is approximately 14 years old (same goes for their classmates Mimihime, Shiro, Anzu, and Taka), Kuku the frog-like girl is approximately 10 years old, and Kona (the kids who draws the strange pictures) is approximately 17 years old. After Haruki blacks-out around the half-way point of episode 3, we are treated to an intense sequence/montage (that includes quick cuts of the brain transplant surgery being performed). That particular sequence was a nearly completely anime original scene with the small of exception of a shot (which occurs immediately after Haruki narrates “My head…it’s so hot”) of what is presumably a POV memory from Kiriko looking at a sleeping Haruki, this was adapted from a similar single out-of-context panel from chapter 33 of the manga. Also immediately after the memory shot of Haruki sleeping, we quickly see what is presumably another POV memory shot of Kiriko, where we see her hands intimately intertwined with another person’s hands, most likely it is a past memory of Kiriko having sex with someone (as to who Kiriko was being intimate with I will leave that to speculation, but I can guarantee it was NOT Haruki, Kiriko did not reciprocate and was unaware of Haruki’s incestuous feelings towards her). Also another neat detail with this episode, the ED has been slightly modified, specifically the part where we see a reflection in a puddle of Kiruko brushing their teeth. Now it starts out as a reflection of Haruki before transforming into Kiruko. Also those with a keen eye for details may have noticed that at the beginning of episode 3 we see a banner/billboard stating that the date is September 12, 2034; and below it is text stating that it has been 10 years since “the calamity”. So we can infer that the year during Maru and Kiruko’s present time is 2039 and that the Calamity took place in 2024 (which is apparently the same year Maru was born). And if anyone is interested in starting the manga after this season is finished, I personally would still recommend starting the manga from the beginning regardless, as in addition to the standard cutting of extra dialogue and scenes from the anime for run-time and cinematic pacing, the chapter titles and cover pages of the manga also play a role in experiencing the foreshadowing of the story that the anime can’t quite replicate (due to the inherent difference in mediums). The mangaka of Heavenly Delusion, Masakazu Ishiguro (who is active on Twitter), their most well known previous work is “And Yet the Town Moves” (also known as Soremachi), which was serialized from 2005-2016. They have also done a number of other single volume manga, a couple of them science fiction stories.

Robin became my favorite character just because he shares the same voice actor as Zoro. This show reminds me of Resident Evil in some ways. The cloning, special abilities, the old lady in the wheel chair that told Tokio about the outside world and the Doc. The lady in the wheel chair reminds of the original Alice and going off of Lupa's theory, the Doc reminds of Alexander Isaacs both from Resident Evil.

You know how at the end Roshi and them said how maybe the facility situation happened before the apocalyptic scenario and how the Doc might be running the facility. If all of that is true then maybe the Doc took the drawings from the kid and actually created the maneaters, which is why at the end we see the fish with hands Maneater that looks exactly like that kid's drawing.

Thankfully this series doesn’t really portray it as something to fetishize/titillate the audience with. It’s more portrayed/framed as a weird/messed up quality of the character, and it being illustrative of Haruki’s emotional dependency (on his sister) and poor socialization (in no small part due to having grown up in a post-apocalyptic environment). So you don’t have to worry about this turning out like a like a lowbrow fetish anime.

It has a manga (currently at 53 chapters, and 49 of those have been collected into 8 tankōbon volumes) and this season of the anime is planned to adapt up to some part of volume 6 (i.e. around chapters 34-37). But to answer your question, that is a no.

My guess is the sister was shot in the head and became brain dead but her body was still alive, people become brain dead all the time. Then the brother was still mentally alive but obviously his body was gone so the doctor put the working brain into the working body. I agree with Lupa that the doctor is most likely at the facility we keep seeing. I think the facility scenes are in the past and the doctor made the monsters, the monsters got loose and destroyed the world.
