The curse went over multiple realms I think. We have Dr Frankenstein in storybrooke and he’s from a different realm. So I guess it takes people that Regina wanted to bring? And she was afraid Cora would find a way to sneak into the curse.

Pretty sure it only took people who were currently located in the enchanted forest. But I think it's a bit like a continent with multiple countries in it. We do see multiple kings/queens co-exist there, and Frankenstein could have been visiting for some reason. 🤷‍♂️
